Form 843 is published by the IRS in its official forms catalog. Download it directly from irs.gov/pub/irs-pdf/f843.pdf. This is a fillable PDF: you can complete it on screen, save a draft, and then print it for the wet-signature filing.

The current revision

The active revision is Form 843 (Rev. 12-2021), indicated in the top-right of page 1. This revision incorporated updates following the 2017 Tax Cuts and Jobs Act and remains current as of 2026. The IRS updates the form periodically — typically every two to three years. Filings submitted on an older revision are generally still processed if the substantive information is complete, but using the current revision is best practice.

Why we don't host the PDF directly

We link to the IRS-hosted version rather than mirroring the PDF here for two reasons: (1) the IRS occasionally issues revisions, and the canonical URL always serves the current version; (2) using the IRS-hosted PDF ensures the digital structure of the form matches what IRS scanners expect when the printed copy is received.

How to complete the fillable PDF

  1. Download the PDF and open in Adobe Reader, Preview (macOS), or a modern PDF editor.
  2. Click each form field and type the answer. The fields auto-format where appropriate (currency, dates).
  3. Save a draft copy with a descriptive filename (e.g., 843-Acme-2020.pdf).
  4. Print on plain white paper, single-sided.
  5. Sign in ink — black or blue ballpoint, no electronic signatures.
  6. Attach the reasonable-cause statement and exhibits behind the form.
  7. Mail by certified mail to the IRS service center for your state. See mailing addresses.

  • Rev. 12-2014 — small format updates
